Nightingale believed that all nurses should work under the guidance of a doctor. Although this belief is still upheld by some today, it does not account for every situation in modern healthcare. In my opinion, there are times when the role of the nurse exceeds that of the generalist and they need to develop their knowledge and skills outside the traditional scope of practice. This paper will explore how far the current role of the professional nurse extends into what was previously thought to be ‘beyond their scope’.
In order to understand where the boundaries lie between the roles of practitioner and professional it is first necessary to examine the history behind the terms ‘practitioner’ and ‘professional’. To do this we must look at the changing roles of nurses within the United Kingdom (UK) and internationally over the last few decades.
The term ‘practitioner’ has its roots in legislation, with the Health Professions Act of 1974 defining practitioners as “a person who carries on any health profession” . Regulation is an important aspect of protecting both the public and practitioners themselves from poor practice; therefore, only those practising healthcare professions properly regulated by law can be called practitioners.
There are many examples throughout history of non-lawful activities carried out by so-called‘practitioners’. As recently as the 1960s in England, midwives were unregulated and could be held liable for malpractice unless they were members of certain organisations, such as the Midwives Society or the General Midwives Council (GMC) . It wasn't until 1993 that all midwife practices became fully regulated, although the law covering these regulations did not come into effect until 1995.
It is not just midwives whose scope of practice has expanded greatly since the 1970s but also that of other nursing specialties
